Please type in SHIPPING FORM information and print the Shipping Form, enter all information on the form and include it with your gauge. Wrap the gauge in bubble wrap and only include the hoses if they need new filters or seals. Please do not send fittings, tools, parts, or the kit case. Ship to address at the top of the form.
